TEAM Foundation Charity Golf Tournament

Chance at Door Prizes Additional fees apply to Mulligans,
and driving contest.
Please Note: If you wish to include a TEAM
employee on your golf team, you must sponsor that employee and include his/her
name on your registration.
Complete and submit registration form by July 1st, 2010.
Sponsorship Opportunities
All sponsors will have their company name posted at an assigned golf hole and
will be recognized the day of the event as well as on TEAM Foundation's website. Bronze
Sponsor - $300
Silver Sponsor - $500
Gold Sponsor - $1,000
Platinum Sponsor - $2,500
Diamond Sponsor - $5,000
